AFTER you complete benefits enrollment in Oracle and are in their system, you must register through Premise Health (formerly CareHere), by following the instructions below:
1. Download the My Premise Health app or go to their website: Premise Health | Care Here - Patient Portal
2. Create your account
3. My Premise Health Portal Assistance options, if needed, are:
- Click the “contact support” button on the website or within the application
- Email mypremisehealthsupport@premisehealth.com
- Call (970) 459-1902
Once on the My Premise Health application, you will see a variety of visit type choices for appointments but please know that if you choose the “wrong” visit type, they will be able to correct it on their end.
